The following outlines the requirements for coaches and officials who are under the age of 18. If you fall in this category, please note the following directions.

• You will need to secure two written references from individuals that would believe that you would not pose any undue risk to participants of USSA. Such references must be provided by individuals who are:

a) not your legal guardian
b) not related to you
c) are at least 18 years old

• These references must be received by USSA within 10 days of membership registration.

Letters of recommendation may be submitted by email by the author to membership@ussa.org, by fax to 435.647.2052, or by mail to the address indicated below.

Upon receipt of your references, USSA will activate your coach/official membership. You will be removed from "pending" status and will be able to print proof of membership from the Member Lookup tool.

NOTE: Failure to provide the required recommendations within 10 days of membership processing will result in inactivation of your membership. Reactivation will require a $25 reactivation fee.
